Metro Community Health Center (MCHC) is an Article 28 Federally Qualified Health Center (FQHC)

(I/DD) Words/Terms to Know

What is I/DD?
I/DD stands for Intellectual and Developmental Disabilities. It refers to a wide range of conditions that affect a person’s intellectual functioning (thinking, learning, and problem-solving) and adaptive behavior (skills like communication, self-care, and social interactions). Children and adults with I/DD may experience delays in development, learning, and independence, but with the right support and services, they can lead fulfilling lives.

Clinic Designation:

  • Article 28: An outpatient medical clinic that provides healthcare services to individuals in accordance with Article 28 of NYS Mental Hygiene Law.
  • Article 31: An outpatient mental health clinic overseen by the NYS Office of Mental Health (OMH), regulated under Article 31 of the NYS Mental Hygiene Law. Provides mental health services to individuals with mental health disorders.
  • Article 16: An outpatient clinic certified by the NYS Office for People with Developmental Disabilities (OPWDD). They provide more comprehensive mental health services for individuals with Intellectual and Developmental Disabilities (ID/D)

Special Education Terms:

  • 504 Plan: A 504 Plan is a legal document that provides accommodations and modifications for students with disabilities to ensure they can fully participate in school activities. This plan is designed for students who may not need special education services but require specific adjustments to the learning environment, such as extended time on tests or a quiet space to work.
  • IEP (Individualized Education Program): An IEP is a legally binding document created for students who qualify for special education services. It outlines specific educational goals, support services, and accommodations designed to help the student succeed academically. The plan is tailored to the individual needs of the student and is reviewed annually to track progress.
  • Psychological Evaluations: These assessments help evaluate a child’s cognitive, emotional, and social development. Psychological evaluations are often used to diagnose disabilities, understand learning challenges, and guide the creation of an IEP or other intervention plans.
  • Psychosocial Evaluation: This comprehensive assessment explores the interplay between an individual’s well-being and the social environment. It aims to understand how these factors interact and influence an individual’s current functioning, behavior, and overall well-being.

Disclaimer: Metro Community Health Center (MCHC) does not provide Psychological and/or Psychosocial Evaluations. We provide psychiatric services like medication management but we do not conduct/perform Psychological and/or Psychosocial Evaluations. You must get these evaluations from another organization.
