Medical Records Requests
Metro Community Health Centers is committed to protecting the confidentiality of your medical records. All medical record information is private and protected under federal and state law.
Medical records requests are generally granted to the patient, an authorized patient representative, or another authorized party such as a physician, medical facility, attorney, court, or insurance company.
How to Request Medical Records
Online Access
Patients can access portions of their medical records through the Healow patient portal. You can message your provider, check lab results, request medication refills, and manage your health information anytime.
In Person
Medical records can be requested in person through your provider’s office. Please bring a valid form of identification.
By Mail
To request your records by mail, complete the authorization form below and send it to:

Before submitting your request, please make sure your form includes:
- Patient name and date of birth
- Name and address of the facility releasing records
- Where the records should be sent
- Dates of service
- Specific records requested
- Reason for request
- Expiration date
- Signature and date
If applicable, please complete all sections related to mental health, substance use treatment, or HIV/AIDS-related information.
For Healthcare Providers
Providers may request medical records by faxing a request on official letterhead.
Please include:
- Patient name
- Date of birth
- Date(s) of service
- Records requested
- Reason for request
- Requested deadline
